How to Register
DUE TO THE CORONAVIRUS CRISIS WE ARE NOT ACCEPTING NEW PATIENTS - WE WILL ONLY BE ABLE TO ACCEPT PATIENTS WHO MAY BE NEW TO THE AREA, WHO HAVE NO REGISTRATION WITH A GP. ANYBODY THAT IS ALREADY REGISTERED WITH A GP AT PRESENT CANNOT MOVE TO THIS PRACTICE.
Our practice list is currently open and we are very pleased to welcome new patients. We are proud to operate a completely non-discriminatory acceptance policy.
If you would like to join our practice please call in and visit our reception desk and ask for a new patient registration pack. An appointment for you to attend for a "new patient health check" will be made. Alternatively please telephone our Reception Team on 01472 353303 for further information.
All we ask is that:
- You reside within our catchment area (see below for boundaries)
- You can provide proof of identity
- That you have not previously been removed from our list due to unacceptable behaviour or
- You attend for a New Patient Health Check
Register Online with the Pelham Medical
You can now register to be a patient with Pelham Medical Group at our Church View site online.
You will need to fill in an application per member of your family who wish to join the Practice. For those patients over the age of 15 years you will be presented with a health questionnaire We ask that you fill in as much information as possible to help us trace your current health records.
Once we have all the details we need a member of the Reception team will contact you to arrange your new patient health check.
Please Note: Our same procedures and policies apply to those patients registering with the Practice online or in person.

All GP Practices are required to allocate a ‘Named Accountable GP’ to all patients. The allocated GP, will have overall responsibility for the care and support that our surgery provides to the patient. However, this does not prevent you from seeing any GP in the practice as you currently do. You are welcome to carry on seeing whichever GP you wish when booking appointments.
